I understand that my photography session is taking place at Glass & Fern Co. LLC, located on private property owned by Mark A. Forry and Katelan D. Forry. The property includes plants, animals, uneven ground, and unguarded bodies of water.

By signing below, I agree that I am voluntarily entering the premises and assume all risks of injury, accident, or loss to myself, my child(ren), or my property.

I release and hold harmless Glass & Fern Co. LLC, Mark A. Forry, Katelan D. Forry, their family, representatives, and agents from any and all liability for injuries, damages, losses, or claims arising from my presence on the property.
